About Seacliff Healthcare
When visits are less frequent, memory is a poor record. A GP Management Plan puts the goals, the medications, the tests and the other professionals involved in writing, so a telephone review or a visit six months later starts from the same page rather than a reconstruction. The plan is written down and shared with you, so a phone review starts from an agreed record. Wombarra is the furthest of the coastal villages we see, and distance is exactly why a written plan matters. Everything agreed is recorded, so a phone review can pick up where the last visit left off.
