About Seacliff Healthcare
Long-term conditions punish reactive care. A GP Management Plan written with you sets out goals, treatment and who else is involved, with review dates booked rather than left to memory. The aim is that the next appointment happens because it is scheduled, not because something went wrong. Review dates are set in the plan itself, so the next appointment is booked before you leave. Austinmer patients can keep review appointments without rearranging the day. A GP Management Plan sets those reviews in advance, so the condition is watched rather than revisited only when it flares.
