A care plan only works if the reviews happen, and reviews happen when the practice is close. From Bulli that is a few minutes each way. We use this approach for diabetes, hypertension, osteoporosis, high cholesterol, migraine, arthritis and back pain. Named conditions are listed publicly, so you know before booking whether a plan applies to yours. For Bulli patients the practice is a short drive south, which makes regular review realistic.
